Servicesadult developmental services Adult Developmental ServicesThe key function of the Community Services Coordination Network is to coordinate access to various services and supports, for adults with a developmental disability. Participation in the CSCN process is required for all adults with a developmental disability who are pursuing access to an accommodation support (residential service). Our role is to link individuals and families to appropriate services and supports in their community. CSCN is intended to augment, not replace, the good planning that is done by service providers across the area. CSCN does not provide direct service. We are part of a planning team that includes the individual, their family and various service providers. When an adult who has a developmental disability wishes to be considered for access to a residential service, an Access Form must be completed and returned to the Community Services Coordination Network. A Local Resolution Team exists in each of the communities in the CSCN catchment area. The Team is chaired by an employee of the Community Services Coordination Network. Members of the Team have a demonstrated commitment to improving the broader service system on behalf of individuals and families. Members are selected who have no known conflict of interest. The group is comprised of innovative, creative thinkers who support the person/family centred planning model. Team members bring a broad cross-section of perspectives to the process including: health, education, employment, leisure, business, community, and service system. The Team is responsible for reviewing the situation of each individual that is referred to CSCN, for access to a residential based service. The purpose of the review is to determine the individual’s level of priority for service. The material that is presented to the Team is anonymous, and contains no identifying information. CSCN is the keeper of the community waiting list for residential services.
